Description

Position Objective: Provides services and supports to individuals in all environments according to each person-centered plan. Recognizes natural learning opportunities and uses those to teach independent living skills, enhancing individuals' independence. To maintain the individuals home within established standards, including health and safety guidelines, protection of individual rights, and fiscal parameters. Will follow Rule 50, Dept. of Human Services Rule 115, Rule 116, Rule 119, Rule 120, and any other relevant regulatory guidelines.

Essential Duties/Responsibilities

  1. Support and educate the philosophy of the agency mission/vision statement and current strategic plan.
  2. Manage all responsibilities based on the agency mission, using individual choice and all appropriate policies, procedures, and regulations.
  3. Delivers services and supports as outlined in each individual's Person-Centered Plan ensuring that activities and supports reflect individual preferences.
  4. Communicates regularly with the Community Living Manager, QSP, nursing, and other team members regarding progress towards outcomes, health and safety, and other issues that may arise.
  5. Assists individuals in maintaining their home, reporting needed repairs or modifications to the appropriate parties within established timeframes.
  6. Recognizes individual differences and adapts teaching/ interacting style to best support the person's needs.
  7. Advocates for individuals through knowledge of personal references, protection from abuse and neglect or other rights violations, and supporting the development of personal relationships.
  8. Follow and implement all pertinent agency policies, departmental protocols and procedures.
  9. Provide required documentation regarding program delivery, medication administration, financial records, and all other requested documentation.
  10. As mandated reporter ensure allegations of abuse and neglect are reported as required.
  11. Perform other duties as assigned.

Education/Experience Requirements

  1. Must be at least 18 years of age.
  2. Posses a High School Diploma or equivalent (GED).
  3. Possess a valid driver's license and automobile insurance.

Competency Requirements

  1. Must communicate effectively, both verbally and in writing.
  2. Mobility to perform assigned duties in various environments including bending, kneeling, standing, repeated lifting.
  3. Ability to perform and maintain certification in CPR, First Aid, CPI, Diet and Nutrition, and Medication authorization.
  4. Knowledge of the principles of Person Centered Planning and the Council of Quality Leadership (CQL).

Physical Requirements

  1. Must be able to lift up to fifty pounds.

Work Environment

  1. Work environments may vary to include home environments and community settings. May be required to work extended shifts and be called upon for emergencies.
