Position Objective: Ensure the delivery of outcome-based services and supports for individuals served reflecting the principles of person-centered planning. The Qualified Intellectual Disability Professional will follow Rule 50, Dept of Human Services Rule 115, Rule 116, Rule 119, Rule 120, and other relevant regulatory guidelines. QIDP will develop individual plans and provide 24 hour oversight and coordination of the habilitation services provided to a resident by Community Option Providers and agency personnel to residents.

Essential Duties/Responsibilities

Mission, Vision, Values

1. Support the philosophy through the agency's mission, vision, and values.

2. Manage all responsibilities based on the agency mission, using individual choice and all appropriate policies, procedures, and regulations.

3. To conduct oneself in a manner that reflects the highest standards of professional courtesy and ethical practices to people served, staff, guardians, families, and all outside contacts.

Database, Paperwork and Compliance Responsibilities:

  1. Meets all deadlines set by the agency and outside resources.
  2. Complete monthly summaries and ensure all documentation is completed to meet clinical and financial requirements.
  3. Maintain files according to standards set by the agency, ICAPs, consents, assessments, and other needed paper or electronic documents.
  4. Review all documentation for accuracy.
  5. Understand rules governing program/licensure for CILA, monitors the implementation to and adherence of these rules.
  6. Complete and required accounting procedures in a timely manner.
  7. Supports database management systems.
  8. Assist with OIG investigations.

Resource Management Responsibilities:

  1. Provide support to and work with all team and agency members.
  2. Maintain positive guardian, family and natural support relations for people supported.
  3. Promote community participation for people.
  4. Review all medical and other incident reports for the person.
  5. Coordinate and support appointments with the nursing department.
  6. Work with ISC's
  1. Assist new providers and individuals in the transition process.
  2. Communicate and serve as an agency liaison to parents, people we support, peers, community, neighbors, and work groups.
  3. Develop and maintain natural support systems and promote community participation for individuals.
  4. Audit Provider compliance with contractual responsibilities and meeting required operational standards during home visits.

Clinical and Behavioral Management Responsibilities:

  1. Effectively facilitate and lead EP!C team meetings so outcomes discussed are clearly identified and recorded.
  2. Implement individual Person-Centered Plans (PCP) for all people on caseload, including writing and entering implementation strategies, assist in facilitating yearly Discovery meetings, along with other meetings as appropriate.
  3. Communicate regularly with individuals served, family/guardian, nursing, and other team members regarding progress toward outcomes, health and safety, and other issues that may arise.
  4. Achieve desired outcomes based on individual choice as delineated in the person's PP
  5. Assist with compatibility activities, admission, transfer, and discharge of individuals.
  6. Provide training (OJT's) and coaching to the contractual providers and agency staff in the principles and practices of Implementation Strategies.
  7. Train staff and providers on Behavior Plans/Interventions related to the person.
  8. Develop behavior plan for individuals who require behavioral support.
  9. Work with all members of a person's team to include but not limited to other EP!C team members, guardians, ISC, BCBA, and psychiatrist.
  10. Audit functions of each contractual provider's home monthly to ensure individual's needs are being met.
  11. Participate in the Professional Review Committee and Human Rights Committee as needed.
  12. Participate in all needed appointments with a person's medical team and psychiatrist.
  13. Participate in the on-call rotation.
  14. Assist with investigations as requested.
  15. Participate in Community Options vacations, power team activities and other events that support the Community Options program.
  16. Participate in the on-call rotation.

Clinical Management Training:

  1. In accordance with agency policy and procedures, support the agency's DSP training in role modeling, coaching, mentoring, and participating in on-the-job training with program staff.
  2. Train all aspects of clinical responsibilities, including incident reports, Personal Plan, monthly summaries, drill sign off, and other clinical tasks.
  3. Provide training and retraining as needed for providers and agency personnel in the efficient use of electronic data entry and communication (Therap & Quik-MAR)
  4. Provide training and coaching to the contractual providers and agency staff in the principles and practices of person-centered planning.

Personal and Professional Development:

  1. Participate in meetings, committees, conferences, seminars, and in-service training sessions as requested.

Personnel Relations:

  1. Interact with all departments and EP!C staff, with individuals from monitoring agencies/committees, parents, family members, and other professionals in a professional manner.
  2. Facilitate intra-and interdepartmental communication on an ongoing basis.
  3. Provide back up to other CO team in QIDP role.

Supervisory Responsibilities:

  1. Requires On-Call responsibilities.

Marginal Responsibilities

  1. Perform other duties as assigned.
  2. As a mandated reporter, ensure allegations of abuse, neglect, and exploitation are reported as required by EP!C policy and government regulation

Education/Experience Requirements

  1. Bachelor's Degree in Human Services required, and two years supervisory experience preferred.
  2. Supervisory experience is preferred.
  3. One year experience with persons with developmental disabilities.
  4. The ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write correspondence.
  5. Successfully complete CPR, First Aid, Infection Control, and Crisis Prevention Institute (CPI) training and maintain these certifications.
  6. Must complete DSP training in accordance with the state regulations and maintain acceptable status on the Health Care Worker's Registry under the Healthcare Worker's Background Check Act.
  7. Must obtain/maintain standing as an Authorized Direct Support Person for medication administration.
  8. Achieve and maintain QIDP status per DHS regulations.
  9. Ability to effectively present information in one-on one and small group situations to customers, clients, and other employees of the organization.
  10. Must possess emotional stability to handle stressful situations on a regular basis and the ability to cope with illness, death, and dying.
  11. Must possess good organizational and time management skills.

Physical Requirements

Work Environment

    1. Work may occur in the following settings: Home, Office, Classroom, Site, Group Homes, or Community.
    2. Participate in a 24 hour on-call system.
    3. Work hours/days are full-time and exempt status. Flexibility is a requirement to accommodate various Program, contractual, personnel and organizational needs.
    4. Will be required to drive a motor vehicle (EP!C's and employee's) as part of his/her employment responsibilities with EP!C. A valid Illinois Driver's License and insurance is required and must be maintained at all times while employed by EP!C.
    5. Be a team player with a positive attitude towards the people EP!C supports, peers, EP!C staff and outside contacts. May be required to work extended shifts and/or be called upon for emergencies.
