Case Manager Permanent Supportive Housing

 

The purpose of this positions is to manage case management functions for assigned permanent supportive housing clients

 

Job Responsibilities

Assess needs (met and unmet), abilities, strengths, barriers and preferences of individuals seeking services and support through the in-take assessment interview

Secure, maintain and conduct regular reviews of program and statistical records through the following databases: Efforts to Outcomes (ETO) and CC/HUD Homeless Management Information Systems (HMIS)

Point of contact program administrator for HMIS data quality and CoC Coordinated Entry Systems

Provide planning, implementation and external supportive services assistance in the following areas: financial education, permanent housing stability, health and wellness, social events, and others as appropriate

Participate in organizational and departmental training and development

Meet external collaborators and members of the Continuum of Care (CoC) on a regular basis to share service provisions and develop appropriate referral/in-take processes 

Job Requirements 

Must be sensitive and responsive to the cultural differences in the organization's service populations

Proficiency with using Microsoft programs

Database experience strongly preferred

Possess strong verbal and written communication skills

Ability to remain calm and responsive while de-escalating crisis situations 

Education Requirements

Graduate degree in Social Work required

Minimum of 5 (five) years' experience working in a nonprofit organization with prior direct client services experience for high risk populations involved with substance use, mental health, chronic health, domestic violence and Department of Corrections
